I, the undersigned, understand and agree that I am entering a martial arts competition of my own free will. I state that I am physically and mentally able to participate in said competition. I agree that ELITE/Joey Perry and anyone associated with said competition is not liable for any injury, dismemberment or hospitalization that may occur due to competing in said tournament. I hereby release ELITE and affiliates from any responsibility for my well being and give permission to ELITE to use any pictures, video or quotes of me for future promotions without further compensation.
